Catching client risk, not just fraud.

Fraud isn't the only blind spot in home care — client risk is another. We're scoping a caregiver-completed social determinants of health (SDOH) check-in: a brief periodic assessment, done roughly every 30 days during a routine visit, capturing signals like housing instability, food insecurity, isolation, and safety concerns.

Rather than sitting in a caregiver's notes and going nowhere, that assessment routes directly to an MCO's service coordination or care management team — surfacing a client sliding toward crisis before it becomes an ER visit or a hospitalization.
